Intercostal Somatosensory-Evoked Potentials and Motor Nerve Conduction Studies in Thoracic Herpes Zoster




Abstract
Intercostal somatosensory evoked potentials(SEP) may be representative of clinical course of thoracic herpes zoster, because the stimulus used for SEP tervels rostrally on ipsilateral posterior collumns of the spinal cord through dorsal root
ganglion
and herpes zoster is a common viral infection of the dorsal root ganglion, Intercostal SEP are not only easily and painlessly obtained, but are specific for individual spinal levels since there is no plexus in thoracic nerve roots.
The purpose of this study was to determine the clinical usefullness of intercostal SEP and motor nerve conduction studies(MNCS) in thoracic herpes zoster.
We studied 27 patients with thoracic herpes zoster on first examinations which were performed from 2 to 45 days(means 15 days) after skin lesion and 19 patients on second examination which were performed from 28 to 118 days(means 64 days) after
skin
lesion.
@ES The results were as follows;
@EN 1) Intercostal SEP recorded scalp and spine showed significantly delayed latencies and decreased amplitudes in involved side as compared with uninvolved side.
2) Motor nerve conduction studies showed significantly delayed latencies and decreased conduction velocities in 4 among 10 cases of lower thoracic lesion.
3) On the second examination, there were mostly improved findings compared with those of the first examination.
4) Among the various SEP parameters, P1 latency difference of cortical SEP between involved and uninvolved side was significantly correlated with pain category scale and visual analogue scale on second examination. And N1 latency difference of
spinal
SEP between involved and uninvolved side was significantly correlated with visual analoge scale on first examination and pain category scale on second examination.
From the above results, intercostal SEP can be useful technique for evaluation of thoracic herpes zoster.
